REMINDER: ALL STUDENT-ATHLETES MUST HAVE THE FOLLOWING ON FILE WITH THE OFFICE OF STUDENT SERVICES AT TMS BEFORE ANY PARTICIPATION IN TRYOUTS AND/OR PRACTICES:

*A PHYSICAL EXAMINATION WITHIN THE LAST CALENDAR YEAR MAY BE TURNED INTO THE ATHLETIC OFFICE, UPLOADED ON SKYWARD, OR EMAILED TO: dsitar@troy30c.org, mdegroot@troy30c.org, or mpeter@troy30c.org

**A sixth grade physical can double as a sports physical if the doctor checks “yes” for interscholastic sports on the back

**All student athletes must participate in all tryouts for their grade/sport to be considered eligible to make the team
